* For 2008-2009 forms, please go to the CIU Online Financial Aid System. After you have completed your 2008-2009 FAFSA, you will receive a CIU Finaid PIN to log into the System.

  • Authorization to Apply Funds -- This document gives the Office of Student Financial Services at CIU permission to credit funds to a student's account. The student will be notified by email after they have submitted their FAFSA if they need to complete this document.

  • Certification Form -- This document is an Institutional Student Information Record that certifies that the student's responsibility in the financial aid process.

  • Dependent Verification -- About 30% of FAFSAs are randomly selected for verification. If a student is selected for verification, they will receive an email from the Office of Student Financial Services asking them to submit a Verification Worksheet. A file cannot be processed for financial aid until verification has been completed. Please note, the Dependent Verification requires a signed copy of both the student's and parent's federal income tax forms.

  • Independent Verification -- About 30% of FAFSAs are randomly selected for verification. If a student is selected for verification, they will receive an email from the Office of Student Financial Services asking them to submit a Verification Worksheet. A file cannot be processed for financial aid until verification has been completed. Please note, the Independent Verification requires a signed copy of the student's federal income tax forms (and their spouse's if applicable).

  • Professional Judgment: Change in Dependency Status -- This form is used to request an adjustment to a student's dependency status for financial aid purposes.

  • Professional Judgment: Change or Loss of Income -- This form is used to request a change to a student's financial aid package due to a loss in income.

  • Professional Judgment: Extended Family Support -- This form is used to request a change to a student's financial aid package due to a change in support from extended family.

  • Professional Judgment: Medical Expenses -- This form is used to request a change to a student's financial aid package due to dental or medical expenses that will affect the family's ability to contribute to toward the cost of education.

  • Professional Judgment: Educational Costs -- This form is used to request a change to a student's financial aid package due to expenses related to private elementary or secondary costs that will affect the family's ability to contribute toward the cost of education.

  • South Carolina Residency Verification -- This form must be completed every year by all LIFE, Palmetto Fellow's, HOPE, and South Carolina Tuition Grant recipients.

  • Loan Return Request -- This form is to be submitted to the Office of Student Financial Services for returning loan money.
