CIU Medical Requirements Health Form
This Health Form must be completely filled out and returned to the Health Services office before you can register for classes. If you have questions about the form, please contact Sally Kennedy at 800.777.2227 ext. 5056 or you may e-mail her at skennedy@ciu.edu.
Please note: Besides insurance information and immunization records, the form requires a TB (Tuberculosis) Test current within the year. Make sure to have the test results listed on the form and have the completed form in by your Orientation Session.
